In recent years, both the Dutch Healthcare Authority (NZa) and insurers have paid increasing attention to correct billing practices. Hospitals and rehabilitation centers have been fined significant amounts, even in cases where there was no evidence of intentional incorrect billing. Even now, there are still (rehabilitation) institutions that have to reimburse large sums to health insurers, and institutions that have been warned by the NZa about potentially incorrect billing. What is going wrong? Is there intentional incorrect billing, or are the complex and unclear funding regulations to blame?

Masterclass on the funding regulations of specialized medical rehabilitation

We frequently receive questions about when treatment time should be recorded, whether a (repeat) outpatient visit is necessary, whether a patient can be referred from the GRZ to the MSR, and whether physician assistants can bill. Can nursing staff also record therapy time, and what impact does this have on the cost of a nursing day?

It’s not entirely clear:

  • How the funding regulations for hospitals with rehabilitation departments relate to the fact that rehabilitation has entirely different DBCs.
  • When physician time may be recorded and how often a rehabilitation physician must see the patient throughout the entire treatment process to be able to bill.
  • How the activities of a different specialist can still count towards determining the MSR DBC.
  • Why activities must still be recorded, even if they do not count towards the DBC.
  • Whether two DBCs may be opened in different institutions for the same patient.
  • Whether a patient may be treated via video conferencing and therefore treatment time may be recorded.
  • What the difference is between a care trajectory and a sub-trajectory.
  • What the relationship is between the cost prices and the maximum rates, and how this relates to agreements with insurers.
  • How travel time should be recorded.
  • How treatment programs fit into the funding system.
  • What the requirements are for the registration of the new patient groups as of 2022.”

Note on Terminology

  • DBCs: Diagnosis-Based Care groups, which are used in the Dutch healthcare system to group patients with similar diagnoses and treatment needs.
  • GRZ: General Rehabilitation Zone (likely a Dutch-specific term related to healthcare regions)
  • MSR: Medical Specialistic Rehabilitation (likely a Dutch-specific term related to a specific type of rehabilitation)
  • Physician assistant: A healthcare professional who practices medicine under the supervision of a physician.
